Kaiawhina (Community Health Worker) – Uawa

TE KAUPAPA O TE TŪRANGA - POSITION PURPOSE

As a Kaiawhina, you will serve as the primarylink between whānau and Primary Health Services. This is a permanentfull-time position (40 hours per week) reporting to the Practice Manager –Coast Clinics. You will work within a multidisciplinary team to deliver mobileservices across the boundaries of Potikirua Ki Te Toka A Taiau.

NGĀ KAWENGA MATUA – KEY ACCOUNTABILITIES In this role, you will be responsible for

  • Whānau Support & Advocacy: Facilitate smooth access and transitions for whānau through health services, advocating for them whenever necessary.
  • Health Promotion: Assist in planning and implementing programmes focused on asthma, diabetes, injury prevention, smoking, mental health, oral health, and     immunisation.
  • Education: Inform and educate whānau and the community about health issues and     general wellness to increase their independence and self-care.
  • Community  Engagement: Work collaboratively with the Primary Health Team to     support target groups from Tamariki (0-14) to Kaumātua (65+).

NGĀ TOHU MATAURANGA - QUALIFICATIONS To be successful, you will need:

  • Community Roots: Established links and networks within the community and a  history of involvement in community activities.
  • Cultural Competence: A high level of cultural sensitivity and an  understanding of Tikanga, Te Reo, and the hapū of Ngāti Porou.
  • Professional  Skills: A medium level of computer literacy (Office 365,  spreadsheets) and excellent communication skills.
  • Qualifications: A clean full Driver’s Licence and full COVID-19 vaccination status  are essential.
  • Growth   Mindset: A willingness to upskill through training, such as Te Reo classes, Vaccinating Health Worker training, or the NZ Certificate in     Health and Wellbeing.
