Te Kaupapa o te Tūranga – Position Purpose

In this Permanent Full-Time role, you will deliver a comprehensive Primary Health Nursing Service across the lifespan, including screening, assessment, education, diagnosis, and treatment for communities within Ngāti Porou. Services encompass practice nursing, well childcare, domiciliary care, palliative care, diabetes checks, chronic care management, health education, promotion, and protection. You will ensure a culturally appropriate, whānau-focused service that meets the needs of whānau, hapū, and communities.

Ngā Kawenga Matua – Key Responsibilities

• Assist GPs and Virtual GP consultations, perform core practice nursing duties, support Registered Health Nurses with wound care and home visits, and manage face-to-face and telephone triage.
• Perform cervical screening with privacy and cultural sensitivity, promote community immunisations, maintain vaccinator certification, and adhere strictly to cold chain management.
• Collaborate with Public Health professionals to deliver community health education programs and conduct health assessments.
• Assist with contact tracing and notifiable disease control, build inter-agency networks, and process external referrals.
• Ensure complete, accurate, and timely clinical data entry using Medtech.

Ngā Tohu Matauranga - Qualifications

Essential:

• Current Annual Practising Certificate.
• Proven experience in Primary Health Nursing.
• Certified Cervical Smear taker and Certified Immunisation Vaccinator.
• Full, clean driver’s licence.
• Strong communication, networking, time management, and organizational skills.
• Pro-immunisation commitment and dedication to continuous quality improvement and professional development.

Desirable:

• Cultural sensitivity and awareness of Te Tiriti o Waitangi and the Ottawa Charter.
• Competency in Medtech 32.
• Experience in Well Child Tamariki Ora services.
• Knowledge of Whānau Ora models and the Primary Healthcare Strategy.
• Knowledge of notifiable disease management.
• Te Reo Māori proficiency
